WAIVER AND LIABILITY RELEASE

WAIVER

I, hereby acknowledge that I am fully aware of the risks and hazards involved in participating in the classes at The Studio Ligaya LLC. These risks include, but are not limited to, those hazards which are inherent in any physical fitness program or activity, such as slipping, falling, and muscle strain.

As a condition of participating in these classes, I knowingly and voluntarily assume all risks associated with participation, including but not limited to injury to myself. I hereby release, waive, discharge The Studio Ligaya its officers, directors, employees, agents, and instructors from any and all liability, claims, demands, causes of action, damages, or expenses that may arise as a result of my participation in these classes, including any claims that arise from the negligence of any person or entity.

I certify that I am physically fit to participate in the classes and that I have no medical conditions that would prevent me from safely participating. Furthermore, I agree to follow all safety instructions given by the fitness instructor and to notify the instructor immediately if I experience any discomfort or pain during class.

LIABILITY RELEASE

I acknowledge that I am participating in fitness classes or activities offered by The Studio Ligaya voluntarily and of my own free will. I understand that there are risks associated with any physical activity or exercise program, and The Studio Ligaya has instructed me to consult with a medical professional prior to starting any fitness regimen.

Therefore, in consideration for the opportunity to participate in The Studio Ligaya’s fitness classes, I agree to assume all risks associated with my participation in fitness activities, and I hereby release and discharge The Studio Ligaya, their respective employees, agents, independent contractors, and all other participants from any and all liability, claims, demands, actions or causes of action of any sort, for any injury, including but not limited to death, that may occur to me or any third party, as a result of my participation in any activities or use of The Studio Ligaya.

I hereby acknowledge and agree that neither The Studio Ligaya nor any of its agents or employees are responsible for any injury or loss of personal property that I may incur while participating in any activities at The Studio Ligaya.

I agree to indemnify, defend, and hold harmless The Studio Ligaya, its agents, employees, and independent contractors from any and all claims, demands, damages, costs, and liabilities, including reasonable attorney’s fees, that arise out of, or are related to, my participation in any activities at The Studio Ligaya.

I hereby certify that I am 18 years of age or older, or the parent or legal guardian of the participant, and have carefully read and understand the contents of this Liability Release Form. I on my behalf, my heirs, successors, and assigns, intend this to be a legally binding instrument.

PHOTO CONSENT

I authorize and consent to the use of my visual by The Studio Ligaya LLC for appropriate purposes, including but not limited to: still photography, videos, electronic and print publications and websites. I give this consent with no claim for payment. 

ACKNOWLEDGEMENT

By way of submitting this form, I certify that I have fully read and understand the waiver  release of liability and photo consent policies and voluntarily agree to its terms.
